values are not getting removed after Form submission

When you mean submit the form and a new form, do you have these forms in a same flow? and also are you sharing the same PVs. What exactly are these two forms and how are they related to each other. What are these forms supposed to do? Please provide all the details.

do you have a process model in which these forms are configured. if so please share the screen shot of same. 

Can you try checking the value of PVs, were the values are modified, what was the value after form submission and before the next User input task was invoked.
