Docx from Dynamic template

Hi,

I am a beginner to Appian .

I am using "Docx from Dynamic template" smart service to read form data (includes images as well)  and generate a document.  Can anyone please help me with the document template sample and xml data model!?

I have also tried MSword 2007 doc from template , pdf docx from template smart services but images were not embedding in the document. Document ID was populating instead of images. Any solution to this issue?
