DRAFT KB-[SP-5430] Connected environment was automatically removed

Symptoms

System Administrators receive an email that reads, "An environment in your infrastructure has been removed", but no system administrators report removing the configuration. 

Additionally, the user who set up the connection is logged in the the devops_infrastructure.csv log as the user who removed the connection:

<Date and Time>,DELETED_REMOTE,<Remote site name>,<Remote Site URL>,<Connection Enabled>,<Remote Connection Enabled>,<Created Date>,<Last Action Type>,<Last Action Actor Name>,<Last Action Actor Username>,<Last Action Date>,<IP address>

Lastly, there is an HTTP 404 at /suite/devops-infrastructure/handlers in the tomcat-access.log of the remote environment:

<Date and Time> /suite/devops-infrastructure/handlers - 404 X.XXX

Cause

When Appian receives a HTTP 404 at the /suite/devops-infrastructure/handlers url, Appian will automatically remove the connected environment.

Action

Re-add the connected environment in the Admin Console.
