System Administrators receive an email saying a connected environment was removed, but no system administrators report removing the configuration.
Additionally the devops_infrastructure.csv log shows the user who setup the connection as the user who removed the connection.

Lastly, there is an HTTP 404 in the tomcat-access.log at /suite/devops-infrastructure/handlers in the remote environment:
/suite/devops-infrastructure/handlers
<Date and Time> /suite/devops-infrastructure/handlers - 404 X.XXX
When Appian receives a HTTP 404 at the /suite/devops-infrastructure/handlers url, Appian will automatically remove the connected environment.
Re-add the connected environment in the Admin Console.
